WHAT YOU'LL NEED FOR LIVE SCAN?

FORM - Bring your completed "REQUEST FOR LIVESCAN" form for your appointment. This form must be filled out correctly and must include the following:

• Full name, CA ID Number (if applicable), Current mailing address, and full Social Security Number. 

If you have not been provided with this form, request one from your referring employer, licensing volunteer organization, or referring agency. Community Care Licensing forms MUST be provided by your employer as they have specific Organization information that cannot be provided by the Livescan Tech. Please feel free to call us for more information.

FORM OF IDENTIFICATION - Bring a valid photo ID which may be one of the following: CA Driver's License, CA DMV ID card, Out-of-State Driver's License, Passport, Alien Registration Card (green card), Military ID card (not expired) or Mexican Consulate ID card.

WHAT YOU'LL NEED FOR INK FINGERPRINTING?

FORM - Bring your completed "FD-258" form for your appointment. The following information will be required on the ink card:

• Requesting ORI Code, Full name, Sex, Height, Weight, Eye color, Hair color, Date of birth, Place of birth, Residence of person fingerprinted, Signature of person fingerprinted, and Social security number.

If you were not provided an "FD-258" Ink Card, you will be provided one by the Fingerprint Roller, however, we WILL NOT be able to provide the Requesting ORI Code. This information must be provided by the agency requesting the ink fingerprint card.
